Youth Waiver and Code of Conduct Form Parent/Guardian Name* First Last Participant's Name* First Last SCAPC Waiver and Code of Conduct for Montreat*I hereby give my permission for the Participant to attend and participate in the Montreat Conference Trip (the “Trip”) conducted through St. Charles Avenue Presbyterian Church (“SCAPC”), and I hereby authorize SCAPC to provide all necessary transportation for this Participant by SCAPC’s Youth Director, chaperones, representatives, and/or volunteers. I hereby acknowledge that I understand the schedule of events and know the costs involved, if applicable. I also acknowledge and agree that the Trip presents inherent risks, including the risk of accidents and/or illnesses, including, but not limited to the risk of contracting COVID-19. In the event of an accident and/or illness, I acknowledge that it may be necessary to proceed with medical and/or dental treatment for the well-being of the Participant and I hereby authorize SCAPC’s Youth Director, chaperones, representatives, volunteers of SCAPC and/or persons acting upon the instruction of any such person to transport the Participant for the purpose of obtaining medical and/or dental examination and treatment and I further hereby authorize the attending physician, surgeon, medical assistant, dentist, and/or other medical professional to exercise their professional judgment to assess the risk incident to the illness or injury and to choose and provide the necessary treatment for the health and well-being of the Participant including, but not limited to, the administration of examinations, diagnostic tests and medication (including anesthesia), blood transfusion, performance of surgery, and any and all medical and/or dental care or treatment deemed necessary, as though I personally was giving them my full approval and support. I further agree to assume responsibility for all medical bills resulting from any such medical treatment. Should it be necessary for the Participant to return home prior to schedule as a result of accident or illness, disciplinary action or otherwise, I hereby agree to assume responsibility for all transportation costs. I hereby release, forever discharge and agree to hold harmless SCAPC, its Youth Director, members of its permanent and interim staff, Session members, chaperones, representatives, and volunteers from any and all liability, claims and demands, in the event of personal injury, sickness or death, as well as property damage and expenses of any nature or kind which may be incurred by the Participant and/or the Undersigned Parent/Guardian in connection with the Trip. Additionally, by signing this agreement, I fully acknowledge and understand the specifically contagious nature of COVID-19 and its related risks and I hereby knowingly and voluntarily assume the risk that the Participant -- and, in turn, his/her/their friends and family with whom he/she/they may be in contact – may be exposed to COVID-19 during this Trip, which may result in personal injury, illness, permanent disability, and death. I, the Parent/Guardian, acknowledge and agree that such risks may result from my own actions, omissions, or negligence, as well as those of the Participant or others, including but not limited to the Youth Director, chaperones, representatives, volunteers, and/or, other persons encountered during the Trip. I hereby voluntarily agree to assume all of the liability and damages and covenant not to sue SCAPC and/or its Youth Director, chaperones, representatives, and/or volunteers and/or any member of SCAPC’s interim or permanent staff and or any member of SCAPC’s Session if COVID-19 is contracted by the Participant and/or then passed on to his/her/their peers, family, or other loved ones. Moreover, I commit that the Participant will abide by the following precautions: • Participant, with his/her/their guardian(s), shall honestly assess his/her/their health the night before the trip through the accompanying questionnaire distributed by SCAPC, and will only attend the Trip if all answers are “no.” • Participant, while on the Trip, shall wear a mask at all times whenever indoors at a public location, and whenever in the presence of non-SCAPC persons. • Participant shall wear a mask and/or comply with other safety measures whenever Montreat personnel or the Youth Director/chaperones instruct him/her/them to do so. • Participant understands that the failure to follow any of the foregoing precautions will disqualify him/her/them from participating or further participating in the Trip. Finally, I agree that, because each participant is representing St. Charles Avenue Presbyterian Church, Christ, and his/her/their own families, he/she/they will conform to these rules of conduct: • Respect one another, staff, and adult leaders (of SCAPC and Montreat). • Participation with the group is expected. • Respect the Church’s and other people’s property. • Respect and comply with event schedules and rules. • No possession or use of alcohol, drugs, or tobacco. • No fighting as well as no explicit or derogatory language towards others. • No harmful or dangerous tools or substances, including weapons, fireworks, lighters, or explosives. • No boys in girls’ sleeping quarters, and no girls in boys’ sleeping quarters. Students who fail to comply with the rules of conduct immediately above may be sent home at their parents’ expense. By executing this waiver, the undersigned acknowledges and agrees that he/she/they hereby release SCAPC, its Youth Director, chaperones, representatives, volunteers, all members of SCAPC’s interim and permanent staff, and all members of SCAPC’s Session from any and all liability for injuries, illnesses (including COVID-19), and/or other damages or costs related to the Trip.
